Understanding Affidavits of One and the Same Person

Is your name spelled differently on your official documents, like your passport, birth certificate or marriage certificate? If so, you may need a one and the same person affidavit.

What is a one and the same person affidavit?

An affidavit of one and the same person is also known as an affidavit of identity. It is a sworn statement that confirms that the different names found on different documents refer to one person. 

Why would I need an affidavit of one and the same person?

You may need an affidavit of identity if your name is spelled differently on different documents, like driver’s licences, citizenship documents, degrees and diplomas, banking records and more.

Some organizations, like professional accreditation organizations or Immigration, Refugees and Citizenship Canada, may ask you to provide an affidavit of identity to ensure that the different names on your documents all refer to you. 

What information should an affidavit of one and the same person include?

Generally, affidavits of identity should include the following information:

  • Your full legal name

  • How your name is spelled on your documents

    • Include the document type, number, place of issue and date of issue

  • A statement that the different spellings of your name refer to one and the same person, being yourself

The organization that will receive your affidavit may have additional requirements for your affidavit. Make sure to confirm their requirements when you are preparing your affidavit.

How do I create an affidavit of identity?
